Parent Commitment Form
The Parent Commitment form provides Children's Literacy Center with a written agreement from you that you will participate as needed in the Peak Reader® program. We believe strongly that your involvment is imperative to your child's success!
This form, along with the Student Referral Form, ensures your child's entrance into the Peak Reader® Program.
