Parent Commitment Form
The Parent Commitment form provides Children's Literacy Center with a written agreement from you that you will participate as needed in the Peak Reader program. We believe strongly that your involvment is imperative to your child's success!
This form, along with the Student Referral Form, ensures your child's entrance into the Peak Reader Program.
